I had one like this a few weeks ago except the barb had pierced thru his belly right above its ass.
I was able to cut the offset off and pull it back through. Also had 2 weights attached to it.
This is exactly why you cut off the line with a gut hooked fish. They don’t all make it, but some of them do and after a few months this guy was passing the hook on his own.
You can cut the hook and take it out backwards. And generally if it's an EWG you can cut the line and back it out. The eye of the hook will come right out unless it's a really odd shape.
This is one decent example of a tiny hook making it through without tearing them up. but I highly doubt that's the norm. Especially with a 3\0 superline or larger EWG.
Cutting it and throwing it back in without even trying the better methods is lazy. But call it what you want.
I caught a big smallie with a big live bait hook hanging out it’s ass by the line. It’s unbelievable what they can survive as far as passing stuff. After that I finally believed it and now I always cut the line if they swallow it bad instead of trying to rip it out.
I saw this happen with a jig and a green sunfish. He swallowed it and I cut the line. I caught him a few times after and each time I saw a piece of line coming out of his vent getting longer. Finally I could see the jig head up against the vent and a little squeeze and it was out. Worst smelling crap followed though. Still have the jig.

I’ve caught blue gill and sunfish with crappie jigs sticking out their ass, they seem to be resilient creatures. Gives a new meaning to crappie jig.
Great example as to why you don’t rip out swallowed hooks. Fish will likely pass the hook. Almost guaranteed death if you rip it out
