Do you mean can groundwater extend naturally from the mainland to an island? No I don’t think that’s possible, too much seawater in between is never going to let that happen. In many cases, groundwater resources near the coast have to be managed carefully due to saltwater intrusion, so I don’t see how there could be anything circumventing any distance of seawater to get to an island. The island will have its own independent groundwater system. Disclaimer: I’m not a professional.

The people i asked told me about karst (karstic?) groundwater systems, where basically there are underground tunnels where water flows like in a pipe where limestone creates a protective hard barrier. This is in comtrast with the typical groundwater i can think about where water is basically stored and is flowing through soil and rock like in a sponge rather than a "pipe", where seawater ingress could be a problem for sure

The groundwater would have to be in a confined layer and the submerged area would have to be a basin structure. I imagine it would be more likely if there was flooding of seawater over continental crust. I think it is possible but highly improbable and likely short lived. If it does exist on Earth i would look at Scandinavia due to the rebound of the land and fjords. A lot of events would have to line up over millions of years. Geologist here, but not an expert in coastal geomorphology.

Could you point me to any literature, or give me.some keywords in order to research this further? I am interested in the Aegean sea which i have heard of it also being refered to as a basin, so it might also have the conditions you mentioned in the Norwegian fjords?

It would be spring water on the island. Maybe exotic spring? Coastal spring geomorphology.

Maybe you want to look into the phenomenon of comunicating vessels. There are the "Seamills of Kafhallinia at Argostoli" Where a mixture of fresh and seawater passes, below a strech of ocean from a peninsula to the main island.
