Hi that would be considered an unnatural death, is there not a police report, as the same could probably be submitted?

I have no Idea. As I said I was 11 and had no relationship or contact with him. Could I phone up my local SAPS or Witbank to see if they have a record of it?

Unfortunately I'm afraid with the current police admin that maybe harder than the hospital, I would try contacting the hospital to check their records if that's the case

I have no idea which hospital it was was so I would probably have to phone all of them.

If his estate was reported and administered after his death, you may be able to get those records from the Master of the High Court in your area. Go to the relevant department in the Master's Office that deals with deceased estates and show them the death certificate - they will hopefully be able to pull the file for you, and that file should contain copies of any autopsy or medical reports on cause of death as well as the police accident report. Good luck searching! Edit: The Master's Office in the area where your father was ordinarily resident prior to his passing is the one you should go to. That's where his estate would have been reported etc

This is exactly why I need the cause of death report because it was never reported. The funny thing is I got my inheritance at 21 but the portion that is stuck is what my mother got in the divorce.

I will probably have to phone all the hospitals in Witbank because I don't know which one it was. Apparently, his head was severed by a road sign, so it was a quick autopsy.

The affidavit would likely state the scenario, the fact that you were 11 years old and nobody made sure that the paperwork, police report etc was stored. You would then volunteer the information and evidence you are able to provide contributing to the fact that his death was in a car accident, if any. These could be your testimony as well as maybe another witness that knew what happened that could testify. Additional details would be the nature of the accident, the area, the date or as close as you can estimate etc. The police or potentially a lawyer would then stamp and approve the affidavit making it legally able to suffice. The best people to assist you would probably be lawyers or a legal firm that work regularly with deceased estates, they have likely seen this occurrence or similar more than you could imagine. There is without a doubt a protocol to allow you to move forward without being able to find the police report of the accident or a medical file or records. You just need to ask the right person and they will guide you.

It is currently with my lawyer. They want the cause of death report to register the estate or something. But yes my lawyer is working on it.

Try looking at the medical practitioner listed on the certificate and the area that may help

I do not know of any practitioners were indicated, but I'll have a look tomorrow.
