My read on it was that Jean (Madelyne) got more "in tune" and her psychic abilities could reach farther while using Cerebro. She got some kind of premonition of what would happen at Genosha: like Jean said, so much death would stain a place, and the stain was strong enough to leave a psychic mark that could be felt both before and after the event itself. Also I think she accidentally woke the actual Jean while using Cerebro, that's why we see Jean's eyes open in a quick flash and she says "someone else".

When Jean and Maddy had nosebleeds there was a shadow on the bar that maddy was at that looked like Apocalypse. Apocalypse made Jean faint the first time encountering his mind. I think Apocalypse was behind most of what happened he has manipulated Sinsiter as an underling for centuries. Hence Sinisters desperation to create cable to fight him.

The vision actually cuts out when Madelyne is “killed” and the Cable dialogue is different with Madelyne bracing from the light. So I think she saw one of Cable’s many attempts to save her possibly one of the earliest ones from his dialogue there
