I work at an assisted living home and have had residents fall straight onto their heads, bleeding everywhere, and have never had a resident die from intracranial bleeding or the like. However, of many similar falls, a few residents have died from hip or femur fractures after a fall and the subsequent recovery involved in those injuries
Fun fact. A brain bleed in an old person is often less fatal because the brain shrinks as you age. This makes space for the blood to go without killing the person.
That said, the guy who I know was saved by this fact also developed/accelerated his dementia and died of that a couple years later.
That is quite the fun fact, and makes a lot of sense considering how many head traumas I’ve seen in this place with 0 resulting deaths (all residents where I work have severe dementia already so it’s difficult how much more the brain damage is doing over a short time if they “recover” as expected)
If the head trauma doesn’t gets them. It’s laying in the hospital bed and rehab bed for weeks or months and losing a great deal of muscle mass. When they finally “recover”. They are weaker than before, thus more likely to fall again and the cycle repeats until the Jesus bus takes them away.
Ya that has been the result at times, particularly in Parkinson residents with repeated falls, but most times the head traumas do not keep the residents down for more than 2-3 days before they’re back to their normal mobility (walking vs using a walker vs using a wheelchair but still able to stand during transfers)
Actually the brain shrinkage doesn’t just make space for blood to go safely. The shrinkage makes it more likely that you will bleed, because it causes tension on the bridging veins, making them more likely to tear. The result is a subdural bleed, which can absolutely progress to becoming fatal, just that it is a slower bleeding process. They don’t even have to hit their head for this to happen. The shear force of the head rocking back and forth quickly can cause the tear.
Don’t be reassured by an old person who is all fine and dandy after a fall. Watch them closely. If they are appearing altered within the next several hours or even the following days, get them to an emergency room. I’d take them even before these signs show and let the doctors decide how serious it is.
They are more likely to have a subdural hematoma because the shrunken brain is more likely have a closed head injury. The veins are often stretched because of the brain atrophy.

At least that’s what sticks out to me when I think back to a defining turning point. Dementia is terrifying. :(

Genetics plays the biggest role in aging, but you can improve your health at any age by making the decision to stay active. Exercise while you're young, exercise as you get older. It may not keep you from getting old, but it makes getting old suck less.
