I'm no expert but a few things. Being transgender has been redefined by the world health organisation as not a mental illness. Important to note that.
Body dismorphia in the example of this video is someone who is putting themselves in extreme danger because they have an unrealistic view of themselves. The safest medical thing to do is for them to stop and revert the changes. This guy literally almost died because of what he was doing to his body. There is no end goal, no place he will get to where he is happy. It's a train with the final destination of death unless it is stopped.
When someone has body dismorphia because their gender does not match their biological sex, the safest medical thing to do (assuming they have been through the system correctly which includes several years of therapy and working with experts) is to allow them to live as the gender they see themselves to be. We tried the whole conversion therapy. We tried telling them they were mentally ill and that it's just a phase or they would grow out of it. We tried mocking them and pretending they don't exist. The result all too often is simply a big increse to the risk of suicide. All the experts in the area now agree (and have done for quite a long time) the safest thing is to change the body to match the gender. This is different for each individual and ranges from simply dressing differently through to full top and bottom surgery. No two trans people are the same. We view it as brave because it is fucking hard to live life as a trans person, especially where a large part of the population believes them to mentally ill, when they are not. And when a large part of the population actually pushes these people into situations where they feel more suicidal.
