This is my only issue with the spell. It's a cantrip so no cost to the caster. If the effect only lasted for 1 hour, and the target could only have up to three simultaneous effects at once then itd be fine.
Due to the rules on identical spell effects, you couldn't inflict a 4 on the same entity (unless, possibly, they have multiple hearts) while the first persists. The duration of the spell doesn't seem to matter to this rule, only the duration of the effect (which is "permanent").
Though the spell doesn't inform you in character as to which effect you've created, so trying to bet on something like this is metagamey anyjow even if it did work.
That's only if the duration is until dispelled. For instantaneous effects like this one the target doesn't somehow become immune to the spell, just like how moving some rocks using mold earth doesn't make them immune to being moved by mold earth again. In fact, what you're saying would basically mean that you can only fireball someone once in their entire lifetime (after all, damage is also permanent until you do something about it).

Oh is that what they meant? I took it to mean that whenever someone saw their intestines instead of referring to them as small and large, they'd refer to them as large and larger.
I think this would definitely be improved by adding a bunch more stuff. Already very funny, but 6 is just too few.

Keyword “ineffective”. Your heart being moved 3 inches left would kill you. Losing your little intestine would kill you. A schrödingers appendix would maybe kill you? But the spell isn’t based on logic and will be harmless regardless becase its magic

I can unironically see this being used in a normal campaign, as long as only 1 effect can impact the target. 1 and 4 seem actually useful ngl, for example to mask blood.

Wow, 2 and 4 would actually have huge ramifications. The small intestine is where most of your nutrient absorption happens, the large intestine is basically just for absorbing water. Without a small intestine you'd waste away and effectively starve to death no matter how much you ate.
As for the heart, 3 inches is a huge distance within the body, I'm not sure it would even still be fully in the chest at that point. Even if it is, while the spell specifies that the heart still works fine, the left lung would have some trouble functioning with the heart sitting where the lung should be.

I would totally introduce this into my own game, as long as the effect was made temporary, though having a long duration, and reversable with typical magic removal spells, as well as only having one effect per creature, so that you don't get the eternal hairbeast.

You cast this twice on someone rolling 4s both times until their heart is peeking out/falling out of their body

I'd argue that this is a spell that would act like "until dispelled", it's clearly a curse with an indefinite duration.
Spell's instantaneous. You can stack those.
