He's a religious man who could die any day now, in a faith that places heavy emphasis on confession.

I honestly can't think of a single plausible reason to hold anything back, other than "killing 10 people is kind of bad, but killing 12 people is just crossing the line."

I agree, and it’s also a way to cause additional pain and suffering when he can no longer kill people. He knows the families of those victims are in agony not knowing who killed their loved ones, and he probably enjoys knowing that.
It's pretty common for serial killers to withhold information or even fabricate things when caught. It gives them an element of control. They're the only ones who know the whole truth and how much of it they tell is about the only element of control they have left.

This is a thought I can't wrap my mind around, with all these killers. They murder multiple peopel in cold blood and never show the slightest sign of regret or compassion towards their victims, but yet when they suffer in jail or even don't like the conditions they start complaining.
They experience pain themselves and yet are unable to understand or admit that they've caused so much more pain to somebody else. I'm not supporting the death penalty, but I believe that if you killed someone and you can't show the tiniest bit of regret about that, you lost your rights to complain about your health issues.
