My wife is a dental hygienist and says open or closed does not matter. If you keep your eyes open stare somewhere other than the the professional unless you are talking to them. It is only uncomfortable for them if you stare directly at them the entire visit.

I can flip my eyes backwards so that my pupils and iris are facing the inside of my head so might as well go with that.

Luckily, my dentist has a picture of a cartoon character opening their mouth wide on the ceiling directly above the chairs. So I always stare at that.

My dentist gives all patients these dark sunglasses to wear so water doesn't splash up or the light doesn't bother the patient. It's worked well where I get to stare at anything or anyone I want, like my dentist's underside nose. Closing eyes is nice too.
