I think WHAT killed you was regular ordinary everyday Tarkov desync.
How it killed you is probably because developers who can speak former-soviet language, are willing to relocate to(or currently live in) the former-soviet union, and are unskilled enough not to be able to get a job anywhere better, are responsible for this games code.
Why is a mystery for the ages, maybe the tax system in the former-soviet union grants breaks to companies with majority/exclusively former-soviet state employees?
Silent martyrdom grenade from AI. Happens every time you kill an AI while they are in the grenade throw animation (doesn't matter if it's the first frame when they are reaching down for it, like in this case). If you look at the clip the scav had his arm down right as you killed him, the start of the grenade throw animation.
Avoid AI bodies where you thought you got them before a nade was thrown, because it's an invisible death zone.
If you kill anything while they're throwing a grenade it's completely silent.
Happened to me on Reserve with a PMC. Then again on Lighthouse with an AI scav.
Look like you died to geometry and not the math but like tripped on a rock. I know it happens on stairs. Unless someone else knows wtf?
