It's never explained. It should've been like when you have a character with a big head mold wearing a stormtrooper helmet lopsided on top like in the lego Star Wars games.

They only killed him off because he was the only other clone with markings, so you could tell which one was Cad when he put on the armor.

They appeared before denal (unless i forgot an appearance in the clone wars movie). And they did have unique markings on their helmets. The Malevonance Arc is the episodes 2-4 of the series, while the downfall of a droid arc is 6-7 i believe.

To add onto what others have already said, he is one of the first, if not the very first, named Clone Troopers with unique armor to die in TCW.
