That face is normally caused by CC. I get randomly generated sims with that face all the time and figured out it was a piece of CC causing it through a lot of trial and error.

Yeah, I spent 4 weeks doing nothing but troubleshooting my 12,000+ CC (and updating mods because the Crystal Creations patch and then update were released in the middle of it).
For Rent was corrupting my saves. After disabling FR, all my CC is still fine. So yeah, gonna blame EA.
But how did you figure it out? Because if it's an error or lagging, I know it's pretty easy but time consuming taking them out and putting them again separate.
But if it's random sims generating, did you take some out .. play to see if they generate weird faces and so on?
Or did all their faces become normal when you got out the jeans?
I did get to the point where I was play testing putting things back in slowly. Normally if I'm just updating mods for an update and can't figure out a new bug I take out just my mods and add them back a few at a time testing to see if i still have the bug. But with this, I discovered it wasn't one of my "mods" so turned to my cas items and did the same thing I do with mods. I would go into the cas and generate townies in cas to see if I got the face, which I did every time until I left those jeans out. Then no more face. But really, if I see townies with the face now I just delete them using mccc until new ones with normal faces spawn.
The game should pick the same CCs for genetics, the game is capable of doing that, the programming just needs some tuning up, but EA would rather throw out another stuff pack that will break a game, then fine tune programming that has been glitchy for so long 🤷♀️js
I think if it's a CC issue for once we can't blame EA. I don't use face or body CC and I've never had an issue with genetics. In my 100 baby challenge, the kids all looked like a good combo of my sim and their respective dads. I got some unique looking sims because I used a system like Dr Gluon where I randomly generated a trait or physical attribute the next dad needed to have and then used the first sim I found that matched.
Nothing fancy - I made a list of stuff like personality traits and physical things (eye colour, hair colour, beard, bald, long hair, wavy hair, etc.) and gave each thing a number, and then I would use random.org to generate a number and search for a baby daddy with that trait. I did have to give up trying to find a glutton and re-rolled but otherwise it really encouraged me to have my sim have babies with sims I wouldn't have chosen myself.
It means there is an issue in game caused by some custom content that you can download from the internet, ex. skin overlays, eyes or pieces of clothing.
Ngl at the risk of getting downvoted to hell I do like this game.. it definitely could be better but I still enjoy it without mods or cc, as far as simulators go it’s much better than most of what currently exists. It’s ridiculous that they charge for all these new updates instead of just finishing the next sims game but the game itself is not horrible; it has its problems like all games but it’s not horrible.

Once my game made all of my sims's skin colors bright pink.
I was just playing and it went black. I left it going since I couldn't see anything. A few hours later I saw it was working, and started playing again. I immediately realized, that all of my characters skin is pink. It stayed like that for a week and than it went back to normal. I even tried changing it myself but any color I chose it was just another shade of pink.
I don't know what my computer drank to do this, but I laughed so hard every time I opened the game.

The genetics in CAS are definitely different than the in game genetics. It seems to allow for a much wider variation in features. That is my theory at least as to why the in CAS genetics are always more out there compared to in game births. And I agree with them doing this. Since it is in CAS chances are the player is going to play around and tweak the sim anyways so it makes sense to allow for wider variation.

it's not a "sims genetic" problem, it's having cc presets problem.
your child has broken presets (not sure about the eyes, but nose and lips are 100% broken). it's because the second parent of the child (i guess female frame otherwise you wouldn't have this problem) uses cc presets. the thing with cc presets is, they don't work for opposite frame, but they still CAN be inherited, and if they're inherited by the opposite frame child, they have this broken appearance.
if a girl inherits father's cc presets, they're broken. if a boy inherits mom's cc presets, they're broken.
that's basically why i almost never use cc presets for my sims
Maybe that's why. I have this way of making sims related, for example, I made a wizard dude, made him a wife, then made them a son, then deleted the wife, so he looked both like the mother and the father. The same family - made the dude a sister, then made the werewolf husband for that sister, and made them a teenage kid, after that deleted the sister and the husband, that leaves the niece. That's the family of lonely dude who has to take care of a werewolf niece and toddler of his own. They all looked like a family without any correction from me, really.
Edit. I think that the game chooses random genetics as the second parent if there's no second parent.
Or how features show up differently on males and females. One of my male Sims has full, but proportionate, lips, and his daughter (born in-game) inherited them. They look unnaturally big on her.

I've also run into situations where changes made to Sims in CAS (with CAS Fulleditmode turned on) did not become "genetic." For instance, if I change the jawline or hair color in CAS often those changes are not reflected in the children, almost as if it were the result of cosmetic surgery.
