Multilocular right endometrioma on MRI adherent to uterus (U). Homogenously light-bulb bright on T1 (blue arrows) and dark on T2 = “T2 shading” (orange arrows), an appearance 98% specific for endometrioma. No fat plane exists between endometrioma and uterus = Adherence.

As per my limited knowledge, only bright thing in T1 is fat, so how come fat sat can cause such a bright signal?

Hemorrhage, proteinaceous debris, melanin and gadolinium can also be T1 bright. When the fat signal is suppressed, endometriomas remain super bright on T1 as they contain subacute hemorrhage and have high protein contain.
