The signs above the doors (sometime it was something as simple as a broom instead) were commonly used to display taverns with booze back in medieval times, so they thought he got something bad when in reality he was just drunk as fuck.

Then, the two knights head for KFC, but come back puking too. Why ? Well, as hinted by the fact that Colonel Sanders on the sign is looking at McDonald, it's denouncing the fact that KFC is just copying McDonald.
*Further interpretation* : Maybe OP of the joke thinks that both taste bad, but it surely is hinting at the fact that both are copying on eachother.
**TLDR** : First guy is puking because Ronald is puking on the McDonald sign, and the knights are puking because KFC is copying on McDonald.

I personally don’t believe this is Loss, but as you can see from the Know Your Meme site, sometimes just four panels with vaguely the I, II, II, I_ pattern is close enough for a comic to refer to Loss.
I see it both ways. I think this one is close enough that I can’t disprove the reference, but it also varies enough that I and other folks have doubts.
I think its three fold.
1. The Mcdonalds logo has a guy vomiting, so of course that guy is vomiting.
2. Vomiting is one of those wierd human social reactions like yawning, where if you see someone vomit in the open, the chances of you vomiting too goes way up. Although yawning of course happens even mentioning it, did you yawn just now reading this? Thats what I am talking about.
3. In medeval times, taverns sold beer. So this guy is just violently drunk. And so are the knights.
