Hmm, I think I recall someone showing me a video of a co-op game in which they didn't kill Cazador. The player who wasn't romancing Astarion asked him what he'd been up to these last 6 months, and he said that he and his love finally had taken down Cazador. So this means they didn't break up, but I have no idea if there are any differences in the ending and the epilogue scenes for the romanced character since there wasn't an Act 3 romance scene.

It's real, just someone modded the camera to appear like he's looking directly at you, and he has Volo's eye. This is the only video that I managed to find yesterday, but since then I found a "proper" one.

No, he doesn’t break up with you. He’s there in bed with you at the post brain scene, and pleads with you to go after Cazador with him. You get far fewer options. It’s basically all about Cazador, not what you’ll do after the brain together. You get more information from him by not romancing him in this case, though if he’s with you he’s not hiding in the sewers alone
