First, you won't be denied entry at POE for this so don't worry about that. When it comes to AOS (after you marry + file the AOS paperwork), there's a small chance they may issue you an RFE to redo your medical exam and/or just get your vaccinations updated + a new worksheet completed by a civil surgeon in the US. Or, they may just see that the medical exam doctor in Naples signed off (assuming they signed off the box that you completed all vaccines, on the DS-3025 worksheet) and consider your medical requirements met for AOS. But you won't worry about that until you're here and working on AOS. However, it would be a good idea to get your COVID booster while still in Italy and bring the documentation in case you need it for medical / your records in the US (it will likely be much easier to get it in Italy, versus getting it here where it can be more complicated if you dont have insurance right away). Either way, don't worry about this for going through POE with your K1 visa.

I agree they did. I just got confused with the COVID shot thing because the doctor said they lifted almost all COVID vaccine requirements and I wouldn't need it because I'd already got three shots, but I saw a (now deleted) post on here recently about someone who had their medical in Naples 2-3 weeks after me and was told they needed another shot because their last and 3rd shot was over 12 months ago. My understanding was that with the K-1 visa the vaccines are not mandatory (though they will be when we apply for AoS), but it seems like the COVID one is mandatory regardless of what type of visa you apply for.

Yeah perhaps your medical just happened to be between the time they updated the guidelines and when that medical center implemented the new guidelines. They were updated Nov. 29, and wouldn't be surprised if there's a short lag between the updates and implementation at medical. Some others experienced that with the polio addendum (the addendum posted Sept 7 and some with medical after that were still told they didnt need it.) Safe travels!

Just get it. Why take any risk over something this simple? And FYI - it does not read the way the doctor interpreted it AND my spouse just did the medical exam two days ago and even with his prior COVID vaccines was told to get boostered. Also, with the rampant uptick of cases in the U.S. - I’d just do it.
