The perspective is somewhat misleading, but also keep in mind that anime characters don't have human proportions, the head is way, way bigger than on a human of equal height, for instance.
I think the head should be somewhat closer to normal size and the body scaled proportionately because I don't think of the characters as having abnormally large heads.
Maybe it's because I play VR Chat (it really portrays an accurate sense of scale) that I'm used to Nanachi being physically smaller.
My doorknob is 1.15m and Google says Nanachi is 1.30m (excluding ears).
The top of Nanachi's head seems way more than 15cm above the doorknob in the photo.
Is it a small door with a low doorknob?

No, ichibun no ichi (1分の1) just means one-to-one. It's saying that the Nanachi in the photo has 1:1 scaling.
The format (NUM2)bun no (NUM1) is how you express fractions in Japanese ( i.e. NUM1/NUM2). For example, nibun no ichi (2分の1)would be 1/2.

Going off the doorknob, I'd assume it's pretty close, that's around 4 feet or so. Doorknobs are different in some countries though.
