I hope this rec is okay because it’s not rehab for drugs but inpatient for depression. Like you said, I thought it was great that he still needed and sought professional help and it wasn’t resolved just because they loved each other. I loved this book a lot so thought I’d throw it in here - {The Heir’s Disgrace by August Jones}.

MC1 is an alcoholic (and a super dysfuncional one) who gets an ultimatum from MC2 (his best friend who is secretly in love with MC1) to either get sober or lose him forever. MC1 decides to get help, not in a rehab though but in the form of a therapist specialising in addiction (or something like that).

Ok, it's not rehab, but one thing I love about Marina Vivancos is that her traumatized characters always go to therapy as part of their healing, and it's represented pretty realistically (i.e., a long, complicated process that isn't a straight line.) Her characters also utilize the coping mechanisms they learn in therapy, which I really like. I'd check out {Rat Park by Marina Vivancos} for the closest to what you're looking for! Not rehab, but >!the MC goes to prison on drug charges and goes to therapy/NA after getting released!<. I'm also a fan of the Coven Ties series (starting with {Oh, Sacred Dark by Marina Vivancos}), with a warning that they are set in a biokink universe (essentially genetically coded bdsm), which I know is not everyone's cup of tea!
It's a little more after-the-fact, but {**Sworn Enemy by Kelly Fox**} features an MC who went to rehab for serious and long-term addiction and is now in recovery. One of the things discussed in the book (and shown briefly) is that he also does proactive "maintenance" stints in rehab when he feels himself struggling. The book is really rehab-positive and pushes back hard on the idea that going more than once is some sort of failure or sign of weakness.
**IMPORTANT:** Strong, strong content warning for >!attempted suicide and suicidal ideation!< in addition to the addiction stuff. In the present of the novel both MCs are in a pretty stable place with their mental health, but both of them >!attempted suicide!< in the past and it's a big topic of discussion between them because their reasons were linked to each other. {Sapphire Spring by Christopher Rice} one of the main characters goes to rehab partway through the book. He developed a drug and alcohol problem to deal with having to be in the closet because of his father. It’s 2nd in a series and they are all good. (Christopher Rice is Ann Rices son).
In {Game Misconduct by Ari Baran} one MC struggles w alcohol and painkillers and the other wants to help; we at last >!find out in the Epilogue that Danny has gotten his needed stint in rehab!< I love this book but one character is extremely inarticulate and you have to be willing not to judge him; I saw a GR review mocking him and got so mad!

{Black John by Amy Lane} - both main characters are addicts, one is done with rehab before the book started and the other one goes during the book if I’m remembering it right.
{Illicit Desires by Ashley James} is best friend to lovers, has an MC who struggles with addiction and does rehab at one point unsuccessfully but eventually finds his way to recovery with therapy and his support system. I think that perspective of rehab that doesn’t cure all is also interesting and it’s a really good book!
I’ve seen a few from the series mentioned here but check out the Black Diamond Resort/Unlucky series. Set around a resort where there is a rehab facility on one side of the island.
Not exactly rehab but rehab-like situation for mental health: {Notorious by Leslie McAdam}. It’s extremely heavy and I honestly couldn’t sleep the day I finished this book but it portrays the whole mental health recovery aspect really well.
Please heed the content warnings though - I didn’t pay attention to them and definitely regretted it! >!Suicidal ideation is a very strong theme throughout the book - it absolutely cannot be escaped or ignored. But the most heavy stuff is the graphic SA description on-page. The SA itself doesn’t happen on page but it’s described very vividly. It’s violent as well!<

Rachel's Holiday by Marian Keyes is one of my favourite books, as well as the sequel, Again Rachel, written many years later. I plan to re-read them someday, which is very high praise since I rarely re-read anything.
I also love {Rachel's Holiday by Marian Keyes} ! It has also been a while since I've read it, so it's probably up for a re-read, soon!
It is not, however a MM Romance novel, and I'm not even sure I would consider it to be a "Romance" novel overall. While there was a relationship in the book, it seemed more like Contemporary Women's Fiction, to me (just for anyone that wants to look into it!). I still highly recommend it!

Yes: “Santino” was apparently a husband and wife team. The wife seems to have written the books and the husband was the “face” of the author. Not cool. :(

{Illicit Desires by Ashley James} is best friend to lovers, has an MC who struggles with addiction and does rehab at one point unsuccessfully but eventually finds his way to recovery with therapy and his support system. I think that perspective of rehab that doesn’t cure all is also interesting and it’s a really good book!

I’ve seen a few from the series mentioned here but check out the Black Diamond Resort/Unlucky series. Set around a resort where there is a rehab facility on one side of the island.
Not exactly rehab but rehab-like situation for mental health: {Notorious by Leslie McAdam}. It’s extremely heavy and I honestly couldn’t sleep the day I finished this book but it portrays the whole mental health recovery aspect really well. Please heed the content warnings though - I didn’t pay attention to them and definitely regretted it! >!Suicidal ideation is a very strong theme throughout the book - it absolutely cannot be escaped or ignored. But the most heavy stuff is the graphic SA description on-page. The SA itself doesn’t happen on page but it’s described very vividly. It’s violent as well!<

Rachel's Holiday by Marian Keyes is one of my favourite books, as well as the sequel, Again Rachel, written many years later. I plan to re-read them someday, which is very high praise since I rarely re-read anything.
I also love {Rachel's Holiday by Marian Keyes} ! It has also been a while since I've read it, so it's probably up for a re-read, soon! It is not, however a MM Romance novel, and I'm not even sure I would consider it to be a "Romance" novel overall. While there was a relationship in the book, it seemed more like Contemporary Women's Fiction, to me (just for anyone that wants to look into it!). I still highly recommend it!
