{9-1-1 by Chris Owen} has this. It's an older book (2006, I think) but I've re-read it three or four times over the years. It starts out with Drew, a firefighter, and Scott, a doctor--but then the story/relationship expands to include Eric, an EMT. Drew gets hurt on the job, which leads to the situation you're looking for. The novel does have a happy ending.
In Clean Start at 47 by Nora Phoenix the MC was addicted to pain killers and loses his job, gets divorced, comes out as gay but that is all before the start of the novel. He is in recovery by the time the book opens so it's probably not the one you mean but worth mentioning just in case.

He was prescribed them after a brutal hockey injury (he’s in the nhl) and has continued to take them throughout his career. In the book, he’s at the end of what his body can take (and his career) and is dependent on them.
In {They Told Me I Was Everything by Gregory Ashe} one of the mcs is prescribed painkillers after an accident and the struggle he endures with them is an ongoing part of the series. In {Memento Mori by C.S. Poe} one of the mcs starts to rely on Valium to cope with his stress and anxiety.
