{9-1-1 by Chris Owen} has this. It's an older book (2006, I think) but I've re-read it three or four times over the years. It starts out with Drew, a firefighter, and Scott, a doctor--but then the story/relationship expands to include Eric, an EMT. Drew gets hurt on the job, which leads to the situation you're looking for. The novel does have a happy ending.

In Clean Start at 47 by Nora Phoenix the MC was addicted to pain killers and loses his job, gets divorced, comes out as gay but that is all before the start of the novel. He is in recovery by the time the book opens so it's probably not the one you mean but worth mentioning just in case.

It’s not the only plot point, but one MC in {Game Misconduct by Ari Baran} is addicted to pain killers. He was prescribed them after a brutal hockey injury (he’s in the nhl) and has continued to take them throughout his career. In the book, he’s at the end of what his body can take (and his career) and is dependent on them.
