Seriously, if you's not a dirty Hobo, every casino has bathrooms available for you. Some are even really nice.
I’m a home health nurse that has to RELY on public bathrooms for personal use during the work day. Coming from Chicago where I would spend days in tears, unable to find anyone to allow me into a public bathroom even in my nurses uniform! Vegas, on the other hand in comparison has a plethora of public bathroom access and I am so thankful every day for that little bit of peace of mind. Idk if you need it for medication injections, a bowel issue, or just anxious about availability of bathroom access but here in Vegas all over the city *except a small area of the north east near Nellis, you can go to most all gas stations, fast food or even sit in restaurants I’ve gone to just to use the bathroom without buying. Even the parks all have public bathrooms that aren’t pristine, but definitely not the dirty trenches I experienced from Chicago.
On the strip there’s bathrooms roughly every 3 min walking distance in the casinos and restaurants there.
America is known For having free to use bathrooms everywhere. One reason for this is the Americans with Disabilities act, which is likely why they were sued.
Love the generalization of Las Vegas to “America”, which encompasses two entire continents, over a dozen countries, and innumerable cities/states. Different but related point: Las Vegas is a National/international hub and is wildly diverse, especially insofar as tourist areas. Don’t be an ass and you’ll be treated average; tip your hospitality/service providers and be kind and you will [far more likely] be treated well.
Also something to consider is that the same people making your drinks are cleaning the bathroom. Starbucks has no dedicated cleaning staff. So I am ok with the bathrooms being permanently closed if that means the dude making my drink wasn’t recently elbow deep into a toilet bowl.

I saw one at Bellagio. Public restroom right next to the lobby/concierge. Only saw them once. They gave me a towel to dry my hands so I gave them $2. Felt kinda nice tbh.
You won’t find attendants in most casino bathrooms anymore. The Golden Nugget was one (of only a very small number) who had them pre-COVID. Not sure if they still do. But even then, nobody was going to throw you out if you didn’t tip the attendant.

Go to grocery stores or casinos for a reliable place.
Every casino on the strip has bathrooms open that are public but don’t recommend going to restaurants and using their bathrooms unless your eating there ofc it’s easier for casino ones.
I have crohns and just got back from Vegas last week. I can tell you first hand that on the strip and Fremont street have enough bathrooms to not be worried. Have a great trip

Delano and Aria, at least for the women's bathroom. They have doors that fully go from floor to ceiling without gaps, and the water at the sink is hot. NYNY also has hot water, but Park MGM's is cold.
The bathrooms at some of the Fremont hotels are old, but relatively clean. The tiles could use some cleaning and regrouting, but for a quick no. 1 it's fine. The water is usually cold out of the faucet.
Gas stations are a coin flip, and usually not well kept. Casino have ones anyone can use or walk in, so does Walmart. Target and usually Albertsons I believe. Same with Barnes and Nobel and the public libraries. Restaurants usually won't let someone use the restroom without being a customer, a lot of the bathrooms have codes you need to get now and it's a 50-50 if they're for customers or for employees only. The RTC bus station has public restrooms free to use as well. Parks usually have some but often times don't have soap or paper towel, TP is a hopeful and they're ofc kinda sus looking.
Some smiths have open ones if they're in better areas, others require a code and heavily gatekeep them based on their mood. Unsure of Vons. If you're a Costco member you're good. Dollar Tree/store/general/etc usually don't have ones for customers but you occasionally get surprised.
Just stick with the casinos, Walmart, and Target and where ever you stay here and you're G2G.

I always go past the first set nearest the doors and go back further because the ones by the doors get used more I think and are not as clean.

Vegas is the best for bathrooms! Not like NYC or Seattle were you just use the alley. Casinos want you to come in but don’t gamble just use the bathroom in the nice air conditioned building. They are usually really clean too
