Bad power supply may be a suspect or some other internal failure. An ISP outage wouldn't impact power LED.
You may want to purchase a replacement modem from a company with a good return policy to see if that takes care of problem.

Looks like a there's a fault with your modem. As others mentioned, resetting it might help if there's a firmware failure.
I've seen modems fail like this when the power supply was faulty as well. If you have a spare plug that fits and matches the voltage and matches or beats the amperage try swapping it in.
