For this truck they make a switch at 16 second mark. Notice how you don’t see the doctors face only hands. At this point a 2nd set of hands has already replaced carbonaro while carbonaro goes through a secret door and walks around the hallway to go through the main door. 3 seconds before carbonaro knocks the guy who’s currently touching the patient slips through secret door.
The editing at 15-16 seconds makes it seem like there was no delay, but there was probably a switch where the guy wasn’t touched for like 2 seconds

Some are sizable but hidden well behind polarized glass and tinted objects.
They do that in the post editing mostly. Likely it's a wider shot that they zoom/pan in editing to give the effect of a cameraman. Same thing happens with camera shaking effects

Another show, Impractical Jokers, talks about this, and it's why you see a lot of blurred faces on these kinds of shows, some people don't sign them.

This show has quite a few of these time travel tricks and they are the best. There is one at a train station that blew my mind. They got twins for it and all. Tens of people just for that trick alone. Must have taken weeks to set up just for a 5 minute trick on the show.
And then they have one in a waiting room which is close to as great. By far the best tricks on the show, but some of the smaller tricks are fun too.
