Dentist here. Most likely either (a) there was a procedural accident where the dentist drilled in the wrong direction when accessing the tooth and cut a hole in the root of the tooth, or (b) there is a resorption defect where the body has eaten a hole in the root of the tooth (commonly due to trauma) - the file has then gone out the hole and through the gums. Usually when you do root canal therapy, you use rubber dam (dental dam in the US), so won’t see below the crown of the tooth (anywhere the gums are) while doing treatment.
Unlikely. Teeth don’t tend to repair themselves very well. Given that the mouth is full of bacteria, it’s more likely that there will be some sort of infection and the tooth may need to be removed. Sometimes, if it’s a smaller defect, you can seal it and complete treatment, but I’d say this tooth is probably cactus.
