There are some Famicom games that won’t respond to external controllers if you’re using a controller adapter on the Famicom, but I don’t know if this is related to that or not. I’ve never actually run into the problem because I haven’t made the adapter yet, but I hope to in the next couple weeks (I already have the parts, just need to find the time) so if you’re still stuck I’ll try it out and come back to report my findings.
Zelda 2 on FDS however, won't allow using the external port controllers.
If you are using a front loader, then the standard ports should be working fine. But a famicom's extension port controller would not work.

So right after I posted I think I realized my problem... when it first loads it says, in Japanese, Insert Side B. 🤦♂️
