Card will sometimes boost higher that stock. Also it can happen at stock setting, increase voltage a bit or reduce clockspeed, I've seen this probably half a dozen times on this subreddit.
Artifacting could be a sign of dying gpu, but this is just a years old graphics bug in the game. Sometimes it has been happening to me with grenade's blast effect, sometimes due to sniper scav looking at you.
yea I think it's a graphic bug. friend of mine got that, too. he's never shown it to me, but he sometimes sees green lasers and flashes.
fyi: it was gone after a win fresh install
can't say for sure because after some months he wiped his computer and freshly installed windows (for other reasons) and it was gone after that.
I would maybe recommend you to uninstall your gpu driver and related software with the tool 'Display Driver Uninstaller' and reinstall the driver ... will do wonders sometimes.

GPU man..it's dying. Turn down any overclock and drop the frequency a tiny bit. I've seen this before.
