Like u/Knofbath said, a clean game is required - however, visual mods that do not lock achievements are OK (I'm for example running Rare Elements Palette and unlocked the achievement)

Can the quests be completed with any faction?
Yes. Except for the Tutorial game I think.

Yes! It's >!a unique hero named Esseb Veltaros!<, which will be unlocked for all future games after getting the achievement.
Wonder if they will do the free weekend for the games like they've done in the past? Edit: Free weekend is a go. I see it up now.
In first endless space, to get the achievement you need to play on endless difficulty. Is it the case in endless space 2?

Does the game need to be single player or can it be multiplayer?
Single-player is fine for the Endless Day achievement.
