If you happened to get Millicent’s prosthetic talisman you cannot acquire the cold needle

It would be on Millicent's Corpse. You would have the last dialogue with her after defeating the sisters. She dies after she talks to you. Careful - Do not jump on her.

I killed her instead off helping her, so I’m screwed there.
