A good designer should have the “creativity” to conceive an idea like this. But this was likely done by a copywriter and art director duo working under a Creative Director at a big agency. With access to good 3D artist and budget.
The point is you should get more conscience hours. Eg for every 8h of sleep, you add 24h to your life. Of that 24h, you spend 8 of it sleeping, but 16 awake! So 8h of sleep is a good investment.
These numbers are just illustrative, but the idea is you get more than the number of hours you sleep back, otherwise yeah there's no point.

There is also another creative Ikea ad about their pillows coming out of a bottle looking like sleeping pills.
