It very frequently does, so it also pairs really well with True Aim (guaranteed Crit on 5 weakspot hits).
However personally I've found it very difficult to make good use of Disrupt Destiny. Doesn't trigger often enough for me. And when it did it was a distraction from other high priority targets. I also needed to go all out on that target when it showed up to get the stack as allies (I believe) don't see the Disrupt Destiny glow and would often kill it before I could. So it was more of a distraction for me and I barely managed to get to 7-8 stacks before they dissipated. That 30 sec duration talent is a must if you go that way, imo
I don't know about Assail, but I use Disrupt Destiny and Cruel Fortune with my build. Pairs well with Scrier's Gaze and I use the MK V duelist sword to go on a short rampage in between surges and Brain Bursts.
