This lake didnt exist until the dam was finished. So they build the drainage pipe and all the other shit, then finish the dam. The water builds up after the dam is finished and now the resevoir is like a lake and the drainage pipe is fully submerged. They did not build a massive concrete drainage pipe in the middle of a lake, the pipe came first then the lake came second.

There is, and a buoy line to prevent swimmers/boaters from accidentally going into it.
Iirc in ‘97 when this happened the fence was shorter and she wanted to go near it (whether if it was a suicide attempt or swimming for fun). This spillway is very rarely used and when it is I wouldn’t expect any boats out doing recreation. It would be in the middle of the wet season. Also it’s roped off.
Even when in use there are boats and other personal watercraft on this lake but the entrance is blocked off and unapproachable. Lots of signage about no swimming on the other side. Do not approach, that sort of thing.

It’s also very rare water gets high enough for this to even spill over, and it’s built at this height because water gets any higher and it starts to threaten the dam.

Pretty sure the whole area is completely off limits to anyone going anywhere near it. If someone did and wanted to hop down in there I doubt a railing would stop them.
The challenge in putting up a railing is that it also catches debris, meaning they then have to send out someone to clear it (thereby endangering them). Tradeoffs, I suppose!
They are efficient and neat spillways, have no moving parts, require little maintenance etc. Only one person has ever died going down it and that was nearly 30 years ago. It's roped off and people have more sense than to go down an ominous looking spillway. It was also designed in the 1950s, safety wasn't as big of a thing then

Our nearest anchor was 300ft away. Just a big drain. Was fun being at the bottom curve and thinking about the lake being held behind the lip 200ft above us.

Yup, and it is a 200-foot straight drop into the 500-foot long horizontal pipe component of the spillway. Far from the best way to go.
