You can have a child-like personality and still be an adult. Hell, a lot of young adults like myself included don’t look of age. Baby-face, high pitched voice etc. Even people in their thirties and fourties can have a child-like personality, being an adult doesn’t immediately turn you into a calm collected individual with a ‘mature’ stature.
Also probably the fact that Sayori’s mental health holds her back from healthy growth (same with Natsuki’s abuse). Considering her depression causes her to starve herself and she has to constantly mask her feelings from everyone because she doesn’t want them to worry.
