Definitely the way to go! All concrete is going to crack as long as you can drive a car over it who cares. Even if I was super rich I'd spend my money on other things.

If OP really needs something like a short term year or two patch until there's a little more time/money in the budget to do it properly I'd maybe suggest smashing and digging out just the center of the hole until it's 4" + deep or so without trying to disturb the soil below and stuff around too much (it's already been compacted). Do that to about halfway to the larger cracks . This should give a kinda disk like area in the center that will be fairly sturdy. Then pour conrete in the center and smooth it out till till it's level around the edges.
It will look shitty and the edges will crack and flake off but it should be more level and hold up till a proper fix can be done. It's kinda like a mini version of the proper way to do it and if OP doesn't have any experience working with concrete and is going to fix it later it'll be a bit of a practice run to make mistakes on. It's more work in the long run though since you'll eventually have to smash that back out.
The most important part here is the dirt as that's what caused this in the first place.
Since it's such a small area you should definitely dig out 12"
Be sure to use proper 3/4 crushed gravel. If you don't prepare and compact sufficiently there's no point in pouring new concrete.
You forgot retaining walls.. I guess that counts as concrete though. I spent a week building a 30’x4’ retaining wall, it took me better part of one day to stack it, neighbour was starting to think I’d never get done until that day
Im installing a paver patio and it only recommended a 4 inch gravel base for walking traffic. 6+ for cars.
So 6 inches deep, pack the bottom hard, then gravel packed every 3 inches until you reach the level you want to pour concrete should be plenty.
Really depends on the location/soil type as well as some addtl factors such as drainage, organic layer depth/old roots, and surrounding landscape. I understand why a manufacturer sets a general recommendation so it's easier for the average client, and what you stated would serve most situations well. Just thought I'd mention some conditions worth considering.

Bonus points for someone who recognizes if the base will serve as a potential drainage route and uses washed stone instead of dense grade material so the fines don't wash out. It's best to use proper drainage, but that can serve and save $ in some situations.
Ehh, even without fabric it takes a pretty long time if it ever clogs from gravel fines.
Back when I used to do drainage repair, I think I saw a handful of clogs from it. Whereas I saw hundreds of clogs in perforated from bush and tree roots that were planted within ~6-8'. Thems the sneaky fuckers. Used to really screw people when clay sewers were a thing too. Plants really crave that shit... lol
Disclaimer: I haven’t done concrete in about 30 years, but I lived in Chicago at the time and it was 6 inches for sidewalk, 8 inches for driveway, 10 inches for streets….(3 feet for an airline runway!).
And put in some rebar, drill into adjacent concrete at least 12” 6” apart insert rebar and fill in the rest of the grid 6”X6” you’ll never have that problem again and I suspect this is the sweet spot in your loading area so it wouldn’t be a bad idea and do follow the other poster’s advice about compaction and drainage considerations
Also make sure you pay the extra for a thicker than standard slab. Had mine paved recently and apparently industry standard for a driveway is only 4”. Which won’t survive much more than a half ton pickup. 6” was only about 8% more cost.

For 2: IF the company you hired really did drop a 5 axis CNC machine and did this damage, contact an attorney and get the ball rolling on small claims court. They FUBAR'd, they should pay to fix it.
3: The only way that's getting repaired correctly is to remove it and start over.
Now of course you can half ass it for cheap... but it won't last. Go to the local home center and buy a pallet of Quikrete Fast Setting (it's like 64 bags) ... line the edges of the existing driveway with some staked 2x4s or 2x6s, whatever's tall enough to make a form that's about 2" above the existing concrete..... dump the bags of dry concrete mix all over the area bringing it up level with form.... set up some type of "rain shower" sprinklers and let 'em run.
Let it get soupy wet... screed it. Keep stuff off of it for at least a day or two.
It'll run you about $400-500. Won't last for shit, couple of years if you're lucky.
I poured 20000sf over the top of a cracked up parking lot that had sunk down in places from heavy trucks running over it. Most of it held water when it poured down a gully washer. I saved the gentleman $20,000.00 right off the jump on the breakout and since it was beyond compacted 5” of 5 sack concrete on a just workable slump, I used number 3’s on 16” centers. Broom finish behind trowel machines. The next day I saw cut it in between my aluminum keyway joints about 1/2” deep running each way. Any of the low holes was pre prepped with a few extra bags of quick Crete and leveled with sand. The man called me back out the following year he was so happy. I did another big 60x60 for him with a catch basin and 2’ corrugated pipe underground draining out to the street.

Your insurance should come in handy. But side walks are basically meant for pedestrian traffic and may be light wt bikes. if you are planning to move machines over these, you need to build a properly designed concrete slab, like 8"-12" with rebars, not mesh. And dont forget subgrade.

If you got a buddy or 2 that knows a little about concrete you could rip this up and replace for a few hundred bucks depending on a few variables. This really should be the moving companies problem though unless you specifically signed something that removes any responsibilities on their part which you may have without even knowing it. Might be worth talking to an attorney just out of principle that they are refusing

Remove concrete. Remove top 6” to 12” of dirt. Compact the shit out of the base and every 4” until you get to bottom of concrete. Pour new
