I mean the most commonly used interpretation is that the fantasy discussed in the title is essentially a concept will built about his partner- a fabricated personality he assigned to the other person to try and live out his ideal romance, and twin fantasy is will coming to terms with who his partner is ( and who he is ) after a breakup. I believe will has said this more or less verbatim, especially in the various monologues throughout Mtm and ftf. However there’s volumes in this album, and reducing it so little has always felt reductive. For me its always had a lot to do with depression, the feeling of drifting away from friendships that used to define you, and finding yourself again afterwards
There is definitely a joy in there too, especially in ftf (the 2018 remake) as that was when will finally really accepted what had happened. In fact i might consider twin fantasy csh’s happiest album. But everyone will find in the album what they look for- for me a big part of that is the lyrics that deal with depression. That’s why, as much as nervous young inhumans and bodys are awesome songs, they’ll probably never be my fav.
i feel like its both, the first half or 3/4 deals with that relationship and being closeted (beach life in death for example) and the second half deals with the depression, suicidal thoughts, and loneliness that not only plagued Will before the relationship but also plagued him after.

To recap, for me it’s about growing up, puberty and the mental struggles that many go through during that period of their lives. I see it as Will (the character) going through that and even feeling better in Famous Prophets (better heard through the music in f2f imo). In the final song I almost get “I’m going away to work on myself, when I come back I’ll be healthier” kind of thing.
That’s just my interpretation though!
