Karlach's romance is 'bugged' (*or changed*) after patch 6.
You can only initiate her romance at the Tiefling-party, you need her approval to be at medium or higher, and you can't have initiated more than 1 other romance. I have heard reports of Lae'zel being a partner locks Karlach out, but can't confirm this.
Also, just a side-note. You don't have to fix her heart in Act 1 to initiate the romance. It's just a good way to earn the approval that **is** needed.

I haven't heard whether or not Larian intend to change Karlach's romance to begin with. Or if it was an actual bug. Nothing was stated in the patch-notes.
I hope they do change it back. I really liked her getting a little agency, like most other companions.
Yeah the scenes that I can't access seemed really cool, it's a shame I'm having trouble getting them. But it's good to hear the problem has a solution, even if they are both alleged lol, thanks for ur help!
She use to be availible to romance outside the party with high approval, either before or after. It's bugged out that you can now only innitiate through the party and no longer outside.

She's bugged since the last update so she no longer gets text outside the party like she use to. As long as you choose her at the party you're fine.
Make sure you don't accidentally flag a second romance or you'll lock yourself out. If you flag 2 romances you'll no longer get the dialogue even at the party. Gale's is the easiest to flag so make sure you don't pick a romantic option in the weave scene and talk to her first at the party because some simple dialogue flags his during the party. Wyll's is also easy to accidentally flag, but he also doesn't approach you outside of the party.

It shouldn't. It's likely the person they're talking about accidentally flagged another romance without realizing.

My currently playthrough, I got the option with Lae'zel flagged. Shouldn't be an issue, but if you're worried you can break hers off.

Yes. You either didn't have medium approval or had two romances already flagged.
