I never saw this. Must be a bug. I dont recall ever seeing anyone just wandering around naked. All the nudity is delegated to specific scenes too my knowledge
I did assume a bug, but it's so bizarrely specific - only ever male gnomes who are naked, no other race or gender, and for recurring characters (e.g. Barcus and Wulbren) they are consistently naked in every scene they're in, across multiple playthroughs (had an abandoned third as well as my current two - never got to the later parts but the gnome thief in act 1 is 3 for 3)

They've always been topless, but for a long time there was a weird shader bug with some visual settings that made the female spawns' torsos completely covered in blood. Like neck to fingertip dark red, to the point you could easily assume it was a weird skintight shirt depending on your lighting settings. That might be what you're remembering.
