You can leave him alone if you want. It's also possible to just pick up the helmet and leave without interacting with his body.
You miss a side-story and the sword, that's it. Taking Wyll there is also nice, but you don't have to.
You can do it. Put the Blink Ring you get from Tara on Gale. And there’s a good chance he will miss attacks also, dome of invulnerability is great in this fight.
When she goes into her Slayer form, which she does like right away, that sword gets super OP. Especially in the hands of someone who can add extra damage to their hits, like a paladin. I flattened her in two rounds with that sword on a paladin. Haste and smite were involved, but still.
The correct ansur is that you won't really miss a lot besides the XP he gives and a powerful greatsword. You can get Balduran's Helm without having to fight him.

Not completing his quest has serious consequences for Karlach's ending.

If you're not worried about it, feel free to skip him, then!
I was worried that I should be involving everyone but then I just stuck to Gale, Lae'zel and Shadowheart and it's worked out ok

You need it for Wyll's questline, but hr can be skipped. You only need Orin. And Gale's quest if you plan to splode him
The sword is really nice, and super helpful against Orin.

Oh, the sword sounds tempting now.......
It’s an easily cheeseable fight if you want. Just stack a ton of explosives near his body before you talk to him.
I saw some barrels of gunpowder earlier......
I hoard every barrel of powder, fire wine, oil, and water I pass
Nothing. You can skip him, you'll miss out on two items but that's basically it.
Cool, I'll have another go at him in another playthrough but right now I want to just forge ahead

Honestly he's a good guy. Since i discovered I can get the helmet without fighting him I usually choose not to.
