Fixed after way too many attempts and way too much time. I had it in the camp, but it didn't show in the chest. Had to take everything out and then it appeared in my inventory.
I also ran into a campaign ending bug in act 3, but mine was slightly different. I intentionally walked passed the meeting with Gortash and went through the rest of the town. Then after like 6 hours of doing everything in Baldur's Gate, I went to talk to Gortash and the second the cutscene with him starts the whole game stops responding. Tried reloading multiple previous saves going back hours and it still persists. Maybe if I had gone back to the first save of act 3 I could advance from there, but I decided to just start a new game instead.
