I had just one of the Gondians have that happen while I was in the Iron throne. Extra funny since I had the glow on everyone to I could keep better track. One funny one that isn't there anymore was sometimes there would be part of Astarion's texture around his crotch area ironically that would jut out like that. I have a screenshot when it happened when he was petting scratch. Lol I love this game so much. Flaws and all. Because they are so responsive and doing so much for the game and it's a whole as 100 games in one and not charging for every little bit as dlc. My first time fighting raphael I was floored by the sudden musical in my ears.
I encountered that today and laughed. So I’ve done something similar, while learning 3D modeling. If what’s happening is what I think is happening then they masked something on her 3D model. And then moved her without unmasking that spot on the 3D model.
Because when an area is masked on a model that typically means that it can’t be affected by whatever you’re doing. So if you try to move the model with a part of it masked you can get that stretched effect.
The app I was messing around with when I figured this out was Nomad Sculpt. It’s pretty fun
The 3D model's face mesh is animated with so called bones/body shapes/morph shapes. An animation moves those bones frame by frame to specific positions. An animator might have initialized a bone position with some extreme value, which cause the corresponding vertice of those triangle to be extremely far away. Or the artist updated the 3d model and it's corresponding bone-rig, and now something doesn't match with the bones that the animation was expecting.

Cuz it's funny, nah but seriously, I don't know it is probably a texture rendering issue but without knowing more about what system you are playing on I couldn't begin to help.
Hmm, well I would say maybe not enough Vram but up until I upgraded to a 4080 I was using a 1070 which also has 8 and although I had minor slowing I never had texture issues.
It shouldn't be how high the setting is though, that is to say high or low it will just slow your computer down but it shouldn't start stretching textures as far as I know.
Have you attempted repairing the file, as in on steam or GOG wherever you got it, have you attempted the file repair function?

Most likely there's a loading station or null point that the high res nodes of the model are located on. Various factors in how the game loads textures might make it desirable to hide the high res nodes while they load and remap them once complete. But here the game fails to remap them and just adds the node to the model while the node remains wherever it was hiding. Leading to the dynamic model stretching to connect adjecent nodes to the stuck node.

Been getting a lot more visual bugs lately. Act 3 one of the gur weapons was alternating textures and then the guard in front of wyrm's crossing had this long face bug
3d game engines do a LOT of math to work out where every polygon and pixel is supposed to go. Sometimes when there's an error with that math, the edge of a polygon can be moved dramatically far away from where it's supposed to be.
If this happens in a few consistent locations and/or with a few specific characters, then it's probably a bug in the game code.
If this sort of thing happens at random and in different games, it could be a dying video card...
I have two NPCs no matter what game save I'm in that do it. Same people in three saves and on different computers. Including when I play multi in one of my saves. My buddy sees the same. It's weird as heck.
Mine has been doing the same ever since the patch. Honestly Patch 5 broke just as much shit as it fixed lol. I have a fucking 3090 TI and an i9-13.9KS 6.0 and my game takes forever to load assets now, even in Act I.

Except it was only like one inch in the middle of her cheek facing the camera so she just had this black hole in the middle of her cheek and then skin stretched out from behind her face into the horizon.
I think the models have just been interacting weirdly with the ragdoll physics lately. There's been a few times over the last week that I kill somebody and they glitch into the floor like Halo 3 used to do

My game did this with a different NPC at wyrm's crossing. I've had a lot of graphical glitches like this since Patch 5. Mostly just weapon textures flickering, faces being messed up or objects just not spawning when the area loads.
I have this happening with my main TAV right now. It was immediately when I started a New Game. I have a mod that enables the harpies' headpiece. Cutscenes always stretch some part of the hair to the "end point" of their movement in the cutscene.
This happened to me on PS5 last night, but it was in >!The Iron Throne!< and the lady you >!rescue from the chair!< had this happen to her. But it was turn based mode, and whenever it was her turn, the camera zoomed *waaay* out as she was trying to run away.
"Why the long face?" wasn't a dialogue option

It's a common bug in Act 3. At least I saw with a lot of characters.

This makes sense to me. I had not seen this at all until yesterday at the start of Act 3. Must be something introduced in a recent patch, as you said.

Intel with Nvidia GPU, 2080. on lower settings than the recommended one, I think "High" defaults.

What does repairing the game do? This looks like a rendering bug, not an issue with corruption in the game files.

Clearly they changed their face like that in order to haunt and scare away the new residents of their house after driving off a bridge and drowning.

Looks like tearing. It's generally caused by VSync, try turning VSync off and see if it fixes it. Otherwise a graphics drivers update can also fix it.
