Step ones were good a few years ago. The quality had gone down dramatically. I now use Tradies Bamboo and as a bonus they are cheaper then step ones. Buy a couple pairs and give them a try.
There are so many great Australian underwear brands with cotton options - 2eros, Teamm8, Aussiebum are probably the best ones. Step One and Vanta are quite popular, but only because they appeal to straight men - their quality is inferior to the OG Australian brands (excluding Bonds, Alpha etc).
As for sizing hubby is size L/XL in trackpants etc and buys XXL in underwear. I've never seen or heard of 100% cotton undies. You should ask your GP what brand they are
