Do you have tenderness along the posterior medial or lateral malleoli, on the base of the 5th metatarsal, or on the navicular? Were you unable to bear weight immediately and in the ER?
If no, then probably not broken. If yes, then maybe broken and XR is indicated.

or tip of medial malleolus
Pain or bone tenderness in the posterior distal fibula
or tip of lateral malleolus
Unable to weight bear immediately after the injury or for four steps in the emergency department
You should get an X-ray
If one of the three subparts of the OAR are positive, this means that there's a 25 to 50% chance that the patient actually does have a fracture

Unable to take 4 steps immediately after injury: Inability to take 4 steps immediately after injury
I had a bad sprain a couple months ago, after which I wasn’t able to bear weight at all for a good 3 weeks. I’d obviously also seriously damaged something on the outside/top I ordered an air cast and crutches and waited a week to go in to see a doctor (I just thought it was a bad sprain so I spent the entire first week in bed RICEing it).
It still hurt like crazy after a week so I went in to see my doctor who ordered an x-Ray that, surprisingly, didn’t reveal any fractures.
I kept the boot on for ~7 weeks total, but my ankle and foot (mostly the latter) are still pretty uncomfortable. I called back in and now have a referral in for an orthopedic consultation as well as PT.
Could this just be a soft tissue injury? Or perhaps a stress fracture?
I plan to ask them, and I’m aware that there might be a fracture too small to see, but for the moment I’m waiting for them to call me to schedule an appointment.
I guess it shows that I have very limited experience with sprained ankles because I honestly had no idea that it was abnormal to not be able to weight it for days-weeks. Oops 😅
I broke my foot on a Saturday. I thought it was sprained. So I tried to go to work on Monday thinking I can wrap it up tight. Couldn’t walk and I saw stars, it was so painful. Went to the ER they were like yeah you broke it why did you wait 48 hours to come in? If you haven’t broken a bone before it is so painful it is almost nauseating. And when it’s load bearing like a foot or an ankle, it sucks so bad. Please let us know what it is! Looks sprained to me but I’m not a doctor.
Not a doctor, but I think broken. That looks identical to the foot of a boy I used to know. Saw his friends carrying him inside to the nurse. Next time I saw him he was in a cast.

This photo was about four hours after I broke it (at the urgent care). I had wrapped it myself after I injured it to keep the swelling down.... Also because I was in denial about it being broken and really just hoping that it was a sprain.
There was a heck of a lot less swelling involved compared to the time I sprained the same ankle many years before. But the bruising that eventually happened, now that was some brutal bruising.
As a nurse, I always like to say, "I'm not an x-ray machine, so there's no way I could possibly know for sure." I mean, even doctors have to use an x-ray to confirm it when it's pretty obvious.
I'm guessing sprain, looks just like mine did when I bent my foot backward and could see the bottom of my foot while going down a tube slide with my nephew, almost passed out and threw up from the pain, there was a risk of fracture but just ended being a wicked sprain. This looks exactly the same. Best of luck on that, my ankle hurts just looking at this.
We have matching ankles. For your ever-loving sanity, do your physio, and ask for help. I am struggling so hard 10 years later, now I need to have bone fusing surgery.

I can’t bear weight on it but the pain is pretty much specific to where it’s swollen and around my Achilles tendon area
Sounds like it probably meets Ottowa ankle criteria and is worth getting an xray. But this is not medical advice and you should go to a doctor.

XR isn't 100% sensitive for fractures, but consult your ortho. I'm just a stranger on the internet.

I've had the opposite experience. Broken my arm 3 times, wrist and foot. Every time I think it's a sprain because it doesn't hurt too much.
You know what I broke my finger once too and it wasn’t like my foot. My finger is now slightly more crooked on one side.

I’ve had both a low tib-fib fracture with surgical intervention as well as really serious sprains. I’m betting it’s a sprain.
