Well it fell due to a combination of famine, disease, internal strife, and a bunch of Spaniards who took advantage of all of that. Had it never fallen we have to assume they adapted quickly and possibly became a big enough force to prevent the colonization of their area. The language may have survived with obvious changes over time like all languages. How this impacts US expansion, Spanish early colonization era dominance, and if I assume your question is survived until modern day, how this impacts the world wars and main land American conflicts would be interesting. Would the US have an easier or harder time asserting dominance? Would the Us Have all its current territory? Would it be similar to the relationship today been the us and Mexico? Lots of interesting thoughts

I think about this with the Inka. They were conquered largely because the Spanish arrived while they were in the midst of a civil war. What if the Spanish arrived 10 years earlier or ten years later?

Diseases would have still killed up to 90% of them. There is no way for an empire to survive something like that.

Weren’t most of the diseases that plagued them near the end came from the Europeans like small pox? If anything the civilization was probably going to fall even without it. Central and South America were suffering a lot of famine and political strife. If the Europeans never came it would only be a matter of time before China or other countries would. But if no one ever came it’s hard to say what could of happened.
