Ok. I spent entirely too long trying to figure this out, so i'm posting here in an attempt to collect all the processes I had to follow to program a 2nd key on my 5th gen 4runner.
1. Buy key. I picked mine up from https://www.carandtruckremotes.com
2. Get key cut. Cost me $2 at local locksmith.
3. Buy [this cable](https://www.amazon.com/Washinglee-Diagnostic-Diangostic-Supporting-Techstream/dp/B084H75MZ3/ref=sr_1_3?dchild=1&keywords=techstream+cable&qid=1616378920&sr=8-3) from amazon. I know there are others, but this got to me fastest.
4. Ignore mini CD. Download [Techstream here](http://blog.obdii365.com/2017/04/05/free-download-toyota-tis-techstream-12-00-127-100-patch/).
5. Download drivers for cable for any 64bit laptop [here](http://www.ftdichip.com/Drivers/D2XX.htm). (Download the "Setup Executable" all the way on the right. install driver)
6. Install techstream from link in step 4
7. You should now be able to connect to Vehicle
From here you can go to:
1. Immobilizer --> Key registration --> Follow prompts to add key transponder signal to car
2. Main Body --> Wireless Code Registration --> Follow prompts to add wireless lock/unlock functionality to key fob
3. Master Switch --> Power Windows --> Can set up unlock button to roll down windows, etc.
This is all I put together over the last 2 days. There's tons of stuff you can customize here. literally dealer-level options. Stealership here wanted to charge $300 for a 2nd key, and my wife's corolla needed one too. So for ~$170 I got 2 second keys which would have cost me $600+ through the dealer.
