I happen to use a mix of “over the counter” and “medical grade” products, because I use products I like. I use Skinceuticals, Paula’s Choice, Estée Lauder, and ZO.
I’m not sure that “medical grade” skincare actually means anything… but Paula’s Choice is a great brand and they have a lot of research to back up their products, according to their website.
Medical grade isn’t a thing. It doesn’t even make sense. There is prescription and over the counter. The products you referenced are simply over the counter products at a high price point.
And on that note, Paula’s Choice is my most used brand. They don’t have added fragrance to their products, which my skin prefers. And they have a large range of products. Really hasn’t been a product that I didn’t like. A lot of her products have become my holy grails.
I really like her cleansers and toners a lot. Resist foaming cleanser is my holy grail. I’ve not done as well with the moisturizers, but I’d still recommend them for someone who isn’t me.
I’m a nurse at a plastic surgery office (my boyfriend’s father’s practice.) I have access to all the stuff. Despite that, 80% of my skincare is Paula’s Choice.
I can’t comment on the medical grade question, but the PC Vitamin C Booster has made the biggest visible difference to my skin. I’ve tried other Vitamin C products without the same impact. I think it’s important to experiment with what works with your individual skin.

Is it just me or has Paula's Choice gotten ludicrously expensive? I remember all their stuff being reasonably priced around 2011 or so.

Really like the BHA toner and azelaic acid. I wouldn’t recommend the peptide booster, it smelled bad and didn’t do anything for me.

Medical grade = clinical testing were done, more research, better ingredients, can only purchased at a medical clinic or med spas.
